During this time, I am accepting a limited number of clients for in-office visits. I have established new rules for clients visiting my office based on guidelines from the Los Angeles County Department of Public Health. If you are not yet comfortable attending in-office visits, telephone and video sessions are available. To obtain in-office services, you agree to take certain precautions that will reduce the risk of exposure. These rules are stirctly enforced, so please review them carefully before scheduling an appointment.
You agree to only keep your appointment if you are symptom-free. If you are experiencing any symptoms of respiratory illness, including fever or cough, please stay home.
I ask that you arrive at my office no earlier than 5 minutes before your scheduled time. The waiting room is currently closed, so you will not be allowed to bring anyone with you. I will text you 5 minutes before your appointment time for office entry.
Masks are required at all times. A new, three-layer filter mask will be provided if you do not bring your own face covering.
You agree to wash your hands or use an alcohol-based hand sanitizer (provided) when you enter my office.
You agree to follow safe distancing precautions. We will keep a distance of 6 feet apart at all times and there will be no physical contact.
If you have or think you might have COVID-19, it is important to stay home and away from other people. According to the Centers for Disease Control, you can be with others after (a) 3 days with no fever and (b) symptoms improved and (c) 10 days after symptoms FIRST appeared.
I'm taking every precaution to limit exposure to COVID-19 by adhering to communicable disease control protocols provided by the Los Angeles County Department of Public Health. You understand that by coming to my office you are assuming the risk of exposure and hold my office harmless of any damages associated with potential COVID-19 exposure.
