Case Study: Hypnosis for Chronic Arthritis Pain


My case study documents nine months of complete pain relief in an 80-year-old client with chronic arthritis, achieved without surgery, medication, or invasive treatment.


An 80-year-old male from New York City has suffered for years with severe arthritis in both feet, with the most severe pain concentrated in his left foot. The pain, sharp and stabbing from the base of his toes to his ankles, had recently escalated beyond what compression sleeves or cushioning pads could manage. At initial presentation, he rated his pain as 7 out of 10 on the Numeric Pain Rating Scale (NPRS). His podiatrist had recommended MLS laser therapy to reduce inflammation, but he faced a two-week wait before treatment could begin.

My Approach


I used Placebo-Enhanced Hypnotic Intervention™, which combines education about placebo research with hypnotic visualization. The approach works in two steps:


Understanding the mechanism: I explained how the brain can activate its own pain control systems, releasing endogenous opioids (natural painkillers chemically similar to morphine) when expectation is properly engaged. Research shows these aren't imaginary effects, they're measurable neurobiological responses.


Targeted visualization: Using hypnosis, I guided him through detailed visualization of the laser therapy he would eventually receive. I had him imagine the laser's therapeutic warmth penetrating deep into his inflamed joints, triggering his body's natural anti-inflammatory response and flooding the area with his own natural painkillers. We followed actual pain pathways from his foot to the brain regions that process and modulate pain. Because he already understood what the laser treatment involved, that knowledge gave the visualization a credible framework, which placebo research associates with stronger responses.

Based on these results, my doctor is holding off on any kind of treatment for the arthritis.

The Results


After the first session focusing on his left foot, his response was immediate: "You helped my foot pain unbelievably. This is like something out of the movies!"


One week later, we focused on his right foot after he reported that his left foot had experienced sustained relief. After a third session targeting both feet, he reported: "It has been about four weeks since I had a session with you and I have not felt pain in my feet since. Amazing!"


He also reported that, after reviewing his progress, his physician elected to postpone the planned arthritis treatment. That decision was his physician's, made independently, and he remained under his podiatrist's care throughout.


Nine months after the first session, he reported his pain at 0 out of 10 on the NPRS, a 7-point reduction from baseline, sustained across 40 weeks.


What This Suggests


The quantitative outcome here is notable: a 7-point NPRS reduction substantially exceeds the Minimal Clinically Important Difference of 2 points commonly used as the threshold for meaningful change, and it held for nine months. Two features stand out. First, the client knew throughout that he was working with visualization and expectation, and this did not appear to reduce the effect, consistent with open-label placebo research. Second, results were reproduced across multiple sessions targeting different areas (left foot, right foot, then both), which suggests a repeatable, self-directed skill rather than a one-time response.

Limitations


This is a single case with self-reported outcomes. It is offered as documentation of one client's reported experience. Hypnosis is not a substitute for medical care, and no one should defer or discontinue treatment recommended by their physician on the basis of this account.
